FMHS Theatre Program Ads - 2023-24

DEADLINE IS THURSDAY, AUGUST 31, 2023

For the ad to be included in the program, you must do three things:
1. Fill out and submit this form
2. Submit payment via PayPal, search @FMHSTheatre
* Please put the student name and "Program Ad" in the memo line (+ What's this payment for?)
3. Submit photos & text in the form below, or email to fmhstheatrepublicity@gmail.com

You can choose to have THE SAME AD in ALL PROGRAMS (all five general theatre productions and the spring musical), or only in the GENERAL PROGRAMS. (You will have the opportunity to add a musical ad later in the year after musical cast and crew has been announced.)

We are happy to create the ad for you with your photo(s) and text. If paying through PayPal, search @FMHSTheatre and please include your student's name and "PROGRAM ADS" in the PayPal memo line. Box 270958, Flower Mound, TX 75027-0958. Payment must be received by August 31, 2023.

Advertising Agreement "Signature" *

It also signifies understanding that if ad content is not submitted electronically by the deadline, it may not be included in the program. All ads must comply with the Lewisville Independent School District (LISD) policies and procedures.
