As the parent or legal guardian of the student filling out this application, I hereby give the nonprofit education agency Dramatic Results permission to teach a virtual class to my student. I understand the Virtual STEAM program is an extension of the ABC Scholars Program and as a result of COVID-19 the agency has had to move its free enrichment activities to an online platform. I understand that Dramatic Results teaching staff will have direct video contact with my child during the class.

My child will be allowed to participate if they are respectful of their teachers and online classmates. Some classmates may be from other Long Beach middle schools. They will follow the same rules about teamwork established within the ABC Program. I acknowledge that my student will be using a computer, chromebook, tablet, or phone provided by our family or Long Beach Unified School District, and that Dramatic Results is not responsible for how students access websites outside of their lessons.

I consent to the use of personal information of my student for internal purposes by Dramatic Results, including video observations to evaluate and improve program delivery. I understand that as an extension of the ABC Scholars Program, which is a federally funded research study, the agency will continue to administer brief student surveys about my child’s feelings and impressions of the lessons being offered. This study will not identify any students by name and all student information will be kept completely confidential.
