Availability in the District Preschool is in extreme high demand.  In order to be fair to all parents and to track requests, we have created this registration questionnaire for Preschool.  By filling this out, it does not guarantee that your child will be placed in a Preschool session, but helps us in our planning.

This is a pre registration form. Please fill out one time only. Students that did not get in will remain on the list. We will call everyone at the end of May to let you know if your child will be coming to preschool that fall. If you have any questions, please call 435-644-6205. Thank you.
