2020 Youth Ministries Registration Form
Complete one Registration Form per teen.

By filling out this one form, a teen will be registered for FBC's 2020 Jr/Sr High Youth Group and 7-12th grade Sunday School.  If you have any questions, please contact the FBC Office at office@fbcmedford.com or (715) 748-4936.  

Sunday School kicks off on Sunday, September 13 at 9:45 AM in room 115 at FBC.
Youth Group kicks off on Wednesday, September 16 from 6-8 PM at the Cutting Edge Youth Center.  

*Please note that we are asking parents and students to wear a mask during the Wisconsin mask mandate.  We will update further as regulations change.
