New Clubs & Organizations Activation Application
Please complete this form to the best of your ability. If you cannot provide the information that is asked (rosters, executive board members, academic advisors), please contact the Student Involvement office regarding why.

When turning in a roster for your club/organization, please submit each members full name, ID number, email address, graduation year, and executive board officer title (if applicable).

Please remember to become an "official" active club/organization on Averett's campus, you must be approved through SGA, have filed a constitution through the Director of Student Involvement, and have a faculty/staff advisor.
