The Big Hook Game Registration Form

Register to play in 'The Big Hook Game' on Saturday 24th October 2020. Join us for a big 11-aside match hosted by Hook Wanderers FC from Hook Church.

The match will kick off at 10:30am (tbc) with the pitch location to be confirmed. We will split all players registered into 2 teams with a mixture of Hook Wanderers players and Hook Church members. Please note there are limited spaces so completing the registration form doesn't guarantee your squad place.

Once you've completed your registration form you will be emailed with confirmation of the team you will be playing for and what kit you need to wear. You will then be required to pay the full £10 match fee.

Please note all players must be 16+ and either from Hook Church, Hook Wanderers or invited friends.

Match Agreement

- I understand that Hook Wanderers is a Christian Football Team as part of Hook Church. (Although you don’t need to be a Christian or have any faith to play in the match, you do agree to respect the Christian values of the team both off and on the field)
- I understand that any injury suffered/sustained or illness picked up whilst playing is my own responsibility and I am playing at my own risk.
- I agree to respect the squad management team at all times.
- I agree to represent the team with the highest standards both on and off the pitch.
- I understand that Hook Wanderers has a zero-tolerance on violence, discrimination and abuse of any form, and agree to follow these rules both on and off the pitch.
- I agree to ensure matchday shirts are always returned after the match. If taken home and lost or damaged I understand I will be charged a £20 fine for the shirt.
- I agree to communicate with the squad manager if I can no longer attend the match.
- I agree to follow all COVID-19 Guidelines and the club risk assessment.
- I agree to pay the match fee by the payment deadline. Failure to do so will result in your removal from the squad for the match.
- I agree to play as a team and to respect all other members of the squad.
- I agree that Hook Wanderers can hold my personal information on file until 14 days prior to the match for Test & Trace purposes.
- I agree that Hook Wanderers can use photos of myself during the match for social media and marketing purposes.
- I agree to pay any match suspension fines I receive in game.
- I agree to inform the squad manager if running late for the match.
- I understand that I may not play the full match. We aim to give every player at least 30 minutes on the pitch, but cannot guarantee this.
- I understand I cannot be refunded for my match fee unless my space can be filled.

I agree to not attend if I have displayed any COVID-19 Symptoms or been in contact with anyone displaying them in the last 14 days. And will make my team manager aware if not. I also agree to let the team manager if I test positive within 14 days of the game.
