We cannot always accommodate all dietary needs or restrictions, but we will try. Please indicate if you have any specific needs *

Boxed Lunch Preference (Sunday) *
Will you require a chair to sit in during the two concerts to be able to participate? (First come, first served) Each devotional concert will last just over an hour. There will be 2 hours between them for rest and a rehearsal. A few chairs (no more than 20) will be available for those who may not be able to stand for about 25 minutes at a time.  (Note: If we have an intermission, no one will have to stand for an hour.) *
Hotel reservations: Please specify if you are staying in the Festival Hotel and if you already have or know of a possible roommate.  If you don't we will work at assigning one to you. *
Please indicate if you will not be riding the provided buses back and forth from the Hotel to the House of Worship. The buses are included in your fee and we prefer that you make use of them. We do not, however, provide handicap transportation so if you need special accommodations please contact rferreira@usbnc.org. We do encourage everybody to use the buses (parking is limited at the House of Worship) *
Payment: We ask that each participant send in a $150 deposit. The remaining balance must be paid by May 10th (Total of $220 dollars). We accept credit cards (Visa, Mastercard or Discover) and checks/money orders. If paying by check or money order, please make it payable to Baha'i Services Fund - Music. Then mail it to: Baha'i House of Worship c/o Van Gilmer 100 Linden Ave. Wilmette, IL 60091. Payments by credit card will be processed by phone & zoom (Rod will reach out to you) For charging on a Credit Card, we will need: The name as it appears on the card/ The card number and The Expiration Date *
