If you wish to sign up to donate blood on Thursday, April 29, 2021, please fill out this form below.

If you are between 16 and under 18 years old, you will need to also have your parents fill out the Parent Consent for Minor Donor form at  https://www.vitalant.org/consent and bring it with you that day. Forms can also be picked up in the library.

Once your appointment is booked, you will receive a text from Vitalant and also from an NHS Blood Drive Team Member.

You will receive a text the day before the drive confirming your appointment time. Kindly respond back to this text so that we know you plan to be there.

The morning of the drive you will receive a text with the link to submit your Fast-Track form. Filling out the Fast-Track before you arrive saves time at the drive.

Students, you will receive a pass on the day of the drive during 1st hour letting you know what time your donation is scheduled for and what time to leave class. You will  need to show your teacher your pass so that you may leave class at the proper time. Please do not show up to the drive before your scheduled time.
