This form is a followup to the recent posting and sharing of the Health and Safety Plan for the reopening of school.

*If you have not reviewed the Health and Safety Plan, please do so before completing this form.

*The form should be completed by parents/guardians on behalf of their student.

*Please complete one form for each student. Students will be enrolled in the learning preference they select on this form. Understanding that this situation continues to change in rapid fashion, we will be as flexible as we can with any requested changes for enrollment going forward.

*If this form is not completed by July 27, 2020, the student will be enrolled in the district's established, in person school model meeting five days per week.

*Questions should be directed to your child's building principal.

Please understand that issues related to the pandemic may at some point force us to return to remote learning by school building or district wide. At this time, Armstrong School District's will be returning to regular school days and hours for the 2020-2021 school year implementing Pennsylvania State and CDC guidelines in the best way possible to support a return and continuation of school. Armstrong School District will have cyber options for students in grades K through 12 that include availability of digital lessons, tutoring by subject throughout the school day, and live streamed lessons with video playback. Students participating in cyber will be issued a Chromebook or laptop in order to complete their school work. Additional information on the district's cyber program is available on the district website.
