Please complete this form if interested in more information regarding services provided by the Student Services Team (SISP). Our SISP team is made up of school counselors (Mrs. Monto Grades K-2, 5) and Ms. Bolumen (Grades 3/4), school psychologist (Ms. Winner), school social worker (Ms. Gadson), school resource officer (Officer Riddick) and School Nurse (Nurse Morgan). Services include but are not limited to counseling support for individual and small groups, family engagement and outreach, community resource referrals, safety, health, wellness and academic intervention supports.

*Teachers/Staff may use this form to make a referral to the SISP team.
