General instructions to ensure children learn and have fun:

1. Teacher/instructional aide, children, and parent(s)/guardian(s) should come with a great attitude.

2. Children are to follow the directions of the teacher/instructional aide.

2. Parent(s)/guardian(s) are to sign in, drop the children off, and remain in the waiting area during the instructional time.

3. Children are to be escorted to the restroom by their parent(s)/guardian(s).

4. To respect the privacy of all children, no photos are allowed.

5. No food or drinks are allowed.

1. No weapons or unlawful drugs are permitted on the premises.

2. No smoking is allowed on the premises except in designated areas (please see the receptionist at the front desk).

Attending any events at the Humane Society of Tampa Bay may require parent(s)/guardian(s), children, and guests to come in contact with animals, and that there are risks associated with these activities, including but not limited to dog or cat bites and/or scratches; Parents, children, and guests knowingly assumes such risks and will not hold Marlene Kanaval, PBJ’s Day School & KidZone, and its instructional aides and associates accountable for any injuries.

4. By attending this event, please know that Marlene Kanaval or PBJ’s Day School & KidZone considers the safety of all children a priority. Your child will be supervised and cared for under such matters as may be called reasonable care for children in the custody of teacher(s)/instructional aide(s) and subject to the teacher’s/instructional aide’s clear instructions. You absolve the teacher(s)/instructional aide(s) and PBJ’s Day School & KidZone of any legal responsibility, above what is considered reasonable, for the safety, welfare, health, and well-being of the above-named child(ren). As their parent(s)/guardian(s), you assume personally and exclusively all responsibility and liability for accident, injury, etc., which may occur to the above-named child(ren) during the time of the event and activities set forth on this permission form.
