This is the 2022 Tunnel Mill Summer Camp Staff Application.

Please answer all questions as best you can.

Any information collected in this application regarding the applicants age or approximation there of is done so to satisfy training and qualifications only and will not impede the applicant from receiving a job for which they are qualified for.

Applicants must be a member of the Boy Scouts of America and/or be willing and able to join.

Applicants must be able to produce the relevant tax and training documentation including, but not limited to: A federal I-9 form and all relevant documentation (ie. State Issued Identification..), A federal W-4 form, an Indiana WH-4 form, Current certification in Youth Protection, Weather Hazard Training, and Sexual Harassment Training.

These documents are due at the time of interview.

Staff member must have an up to date Health form (parts A and B) for staff outings and prior to arriving for camp this summer a part C must be completed.
