Please complete this form to register for a date and time to take the TEAS LPN/RN and AH.  Within 48 business hours after you complete and submit the form, an Outlook Calendar invitation confirming your appointment will be sent to the email you provide on this form.
PLEASE ONLY REGISTER FOR ONE DATE. YOU MAY REGISTER FOR A SECOND ATTEMPT FOLLOWING COMPLETION OF THE TEAS. If you have an ATI Account, use the same name on this registration form that your ATI account is under.

If you have been granted additional testing time from ATI select the 9:30 am testing time. MECC does not have the ability to approve additional testing time for the TEAS, that must be granted through ATI. You do not have to disclose the information to MECC, but will need to select 9:30 am as your test time.
