UB Prehealth Self-Assessment
This self-assessment is designed to help first time applicants to reflect on their strengths, areas to improve, and readiness as a candidate for their chosen health profession. Prehealth Advising is here to assist you through this process. 

Please answer all questions. As this is a lengthy form that cannot be partially saved, you may want to first review it, reflect, and pre-write some of your responses before completing this self-assessment.

Health profession central applications require you to include ALL attempts of ALL courses in your application, even if you retook them. They DO NOT allow grade replacement for retakes. If you retook a course, they will require you to submit both grades and all will be calculated in your GPA and science GPA.

There can be some slight differences in how science GPA is calculated for the different applications, but generally they are looking at what's called the BCPM - Bio, Chem, Physics, Math. 

At UB, courses from the following departments would be included in the BCPM: BCH, BIO, BMS, CHE, MTH, MIC, NRS, PAS, PHY, PGY, STA. 

There are also specific courses from certain departments that can be included in the BCPM: MT 401, APY 345 & 346, APY 348, PSY 207. Please note that no other psych courses are considered in the BCPM. 

With all of this in mind, please use an online GPA calculator (there are numerous available that you can find quickly in a Google search) to calculate your science (BCPM) GPA. 

Remember to include ALL attempts for any repeated coursework.

Letter of recommendation requirements can vary from school to school, but here are some common requirements: 

*3-5 letters.

*At least one letter from a PhD level faculty member who has taught you in a natural science course. Many schools will ask for two of these letters so we would strongly recommend requesting two. 

*At least one letter from a clinically based experience in the profession you are applying for. 

*For anyone applying to a combined PhD program, a research letter is required. A research letter is recommended for other applicants who participated in research. 

*Some DO medical schools may recommend a letter from a DO physician, but it is not often required. 

*Some schools may recommend a faculty letter from a non-science course (humanities and/or social sciences). Please note the non-science letter does not need to be from a PhD level instructor.

*Other letters would depend on what other activities you participate in and could include areas like community service, athletics, leadership positions, and other professional positions. Consider who knows you best in an academic or professional capacity.

*Letters from family or close friends are strongly advised against.

Keeping all of this in mind, please list who you plan to ask for letters of recommendation. For each one, include their name and the relationship/role(s) they have to you (professor, supervisor, research PI, person you shadowed, coach, etc.). If it's a faculty letter, please also specify what course(s) you took with them

All health profession applications ask a question about if you were the recipient of any "institutional action" by a college for unacceptable academic performance or conduct violation. 

You must answer yes even if such action did not interrupt your enrollment or require you to withdraw.

You must answer yes even if you have already completed required community service hours or records have been contained only within a particular department. 

Furthermore, you must answer yes even if you believe that the disciplinary action has already been expunged from your record.
