Touring a local facility is an excellent opportunity for local students and others to learn about the careers in manufacturing and see first-hand “How it’s Made”. Gain insight into the world of manufacturing by choosing to participate in up to three 30-minute tours that will be held on Friday, February 4th.

If you are not 18 years or older, a parent or guardian must complete the form below and provide consent to participate below.  Participants must be at least 14 years of age.

Parent Permission
As a parent or guardian of a student/minor requesting to voluntarily participate in an event, I hereby acknowledge that I have read and understand and agree to the following:
All participants, including accompanying adults, will be asked to wear a mask, sanitize, and wear safety glasses upon entering each facility. All individuals participating will receive safety glasses when they check-in the day of the event. Before entering the facility, individuals will also be covid screened when checking in the morning of the event. All individuals who participate in this event are expected to be respectful of the facilities they are touring and those around them. If any individuals are disrespecting the facility or the individuals within it, they will be asked to leave.
I hereby give permission for the above-named participant (s) to participate in the Manufacturing Day Event on February 4th, 2022.

I understand that participation with this event carries a possibility of risk for physical injury
including broken bones, paralysis or death. I have discussed the potential risks for injury with my child and I believe that my child has sufficient physical ability to safely and voluntarily participate in this event. Further, I agree to save and hold harmless the manufacturers hosting tours and the members of the Grand Traverse Area Manufacturing Committee, volunteers and assigns for any claims, suits or damages (including but not limited to defense and indemnification) which might result from my child participating in the aforementioned event.

I certify that my child has no physical, mental or medical conditions which could interfere with his/her safety in participating in this event.

I hereby authorize the members of GTAMC, volunteers, and emergency medical personnel to examine my child in the event of an accident, injury or serious illness, and to administer emergency care to the above-named student. I understand that the effort will be made to contact me to explain the situation prior to the treatment.

In the event it becomes necessary for GTAMC to obtain emergency care for my student, neither he/she or GTAMC assumes financial liability for the expenses incurred because of the accident, injury, illness and/or other unforeseen circumstances. I understand that I am responsible for any costs associated with an accident, injury, illness and/or other circumstances. Furthermore, I understand that my child must be at least 14 years of age to participate in this event and tour local manufacturing facilities for Manufacturing Day.
