By completing and submitting this application form, you are confirming that you have read the "Information for Counsellors & Terms of Agreement" and accept the terms and conditions.

We will keep the information you provide here for three years after the scheme has ended to support any complaint investigation. We will keep your information on our secure servers and only ACC staff or volunteers working in support of the scheme will have access to it. All those involved in the administration or management of the scheme will be bound by ACC’s confidentiality agreement.

In the event that we pass your contact details to a prospective client this will only be when you have consented to take on that client.
