Submit a separate form for EACH online class you plan to take.  

Once you submit this form, the parent/guardian you enter will receive an email with further instructions. 
  • The email will include a .PDF document
  • This document must be printed, completed, signed and returned to the JHS Counseling Office with your registration worksheet by the due date in order to be registered for the requested online course(s).  
You will not be signed up for an online class until we receive your signed application!
