This survey will be used to help plan for limited capacity church services under COVID-19 constraints.

Please read the following information, about the guidelines that will be in effect for the safety of all, before filling out this survey.

1. All attendees will be required to wear masks and adhere to social distancing guidelines throughout the service.
2. All seating will be assigned.
3. Attendees must remain in their seats throughout the service except when getting up to use the restroom.
4. All attendees will have their temperatures taken before entry into the sanctuary. Anyone with a fever will not be allowed in.
5. No fellowship will be allowed before or after service.
6. There will be no children's church.
7. Restroom occupancy will be limited to ensure adherence to social distancing.
8. Anyone experiencing COVID-19 symptoms such as fever should not attend.
9. Anyone that has been in contact with someone suspected to have or confirmed to have COVID-19 should not attend.
10. Saints over 65 years of age or those at increased risk will be required to sign a waiver of liability in order to be granted entry.
11. By participating in our activities, attendees assume the risk of potentially contracting communicable diseases.
