This form is for any small, outdoor events taking place while Cornerstone Church is in Phase 2 of our Reimagining Life Together plan. If you plan on participating in any Cornerstone sponsored, outdoor events, you must complete this registration form 24-hours prior to attending.

Have you experienced any of the following symptoms that you cannot attribute to a previous health condition: cough, shortness of breath or difficulty breathing, fever over 100.4, chills, muscle pain, headache, sore throat, loss of taste or smell, congestion or runny nose, nausea or vomiting, or diarrhea? *
Have you been been in close contact (spent over 15 minutes within 6 feet) with any person in the last 14 days who has been diagnosed with active COVID-19? *
Have you been been in close contact (spent over 15 minutes within 6 feet) with any person in the last 14 days who is showing symptoms of COVID-19? *
Have you tested positive for the SARS COV2 virus (or been diagnosed with COVID-19) at any time? *
If you answered yes to the previous question, then have you not exhibited any COVID-19 symptoms for at least 14 days and a medical provider has determined you no longer need to be isolated? *
Do you agree to contact your event organizer or the church office if you begin to develop any COVID-19 symptoms within 14 days of attending this event? *
Do you agree to adhere to the following safety guidelines during the entirety of our event: wear a facemask at all times, practice physical distancing of 6 ft. from all non-family members, and sanitize/wash your hands upon arrival and throughout the event when needed? *
