(January 2022) Please note that we no longer require you to submit this form for any NEGATIVE test results, only for void and positive results. We continue to make test kits available to students for self-testing twice a week. (These kits are intended for student use only and not for household sharing.) Kits are distributed in mentor groups roughly every three weeks. Should a student need a replacement kit or not attend mentoring, they can collect a kit from the exams office in A Block.
