Please answer the questions below and submit to confirm your child"s place for surf lessons in Term 1.
Once submitted your Childs place is confirmed.

This enrolment form is for weekly surfing  with Stoked Surf School and is used to gather information such as emergency contact details, special needs etc.

One form per child.

Surfing  -  Mon, Tues, Wed,  Thu After School  & Saturday and Sunday 9am
10 Week program. Commences Tuesday 9th Feb
$25 per session

We meet 10 min prior to the lesson at the Cave Rock end of Sumner beach, big black trailer.

The lesson is for 1 hour and 45 min of organised activities. Sometimes we mix it up with skating, beach flags, movement session if conditions are variable.
If we have to cancel the lesson due to bad weather ( wind, electrical storm) we will email and text you by lunch time day of lesson. A make up lesson can be scheduled on another lesson day during the term.

Terms of payment
- An invoice will be created once the form has been submitted.
- Please pay for the session prior to dropping your surfer off.
- Weekly payment can be organised in advance , please email Nolan
- Unpaid invoices at the 2 week mark will incur a 10% fee

Please NOTE  Only one make up lesson is available per child, non negotiable.If the session is cancelled due to bad weather another day will be offered during the same week or another week during the term.
