This form is for the use of Mid-Ohio Dressage Association to collect information of members. The information is also shared with USDF as a Mid-Ohio Dressage Association is a group member organization (GMO) of USDF. The membership process is complete ONLY when MODA receives complete payment for membership. The membership year runs from December 1st to November 30th with all memberships expiring on November 30th regardless of when a member joins.
