Please complete the form below to register for a booth at the 16th Annual 50 Plus: Challenge to Live Well. We ask you to let us know as soon as possible if you plan to attend, and we appreciate your booth registration form by April 10th.

You may set up your booth Sunday, April 26th at 11:00 a.m. Please bring your own chair. If you require special assistance, let us know.

Please consider providing a door prize and other give-a-way items at your booth.
