2019 UMADAOP Statewide Conference - Vendor Marketplace Application
The 30th Annual UMADAOP State Conference will be held September 11 - 13, 2019,  at the at the Radisson at the University of Toledo,  3100 Glendale Avenue, Toledo, Ohio 43614.  The Vendor Marketplace will be located between the general session and workshop meeting rooms.  Conference participants will have time to visit vendor booths between sessions and at the end of each day.  Cost per table for the three day conference is $25.00 per day and includes a standard 6 foot table; lunch each day; and, marketplace vendors vendors will be featured in the conference booklet.

Please indicate the days you want to participate. Please check all that apply. The vendor fee is $25.00 per day (lunch included). You will receive an invoice and link for payment. Your space is not considered reserved until the fee is paid.  Please note: the times listed are the start and end times of the Vendor Marketplace. No set up will be allowed within 30 minutes of the start time.
