SWAN Food Parcel Request Form
SWAN (Charity No 1188649) has been providing food aid to the local commuity since 2012.

IF YOU ARE IN NEED OF FOOD AID PLEASE COMPLETE THIS FORM AND WE WILL DO OUR BEST TO HELP YOU.

We run a Soup Kitchen at Thornton Heath Islamic Centre every Monday everning at 6.30pm

We run a Foodbank for South London Islamic Centre every Tuesday Morning from 10.30 - 12pm

We have a Community Hub in Croydon from which we run our services.

Please COMPLETE THIS REQUEST FORM and we will arrange a food parcel for you to BE COLLECTED FROM THE FOODBANK, SOUP KITCHEN OR COMMUNITY HUB.

DELIVERIES WILL ONLY BE MADE IN EMERGENCIES OR FOR ANYONE WHO IS SICK, DISABLED OR ELDERLY

We will respond within 48 hours with a message or call to acknowledge your request.
