All requests for athletic fields and facilities must be received no less than 20 days and no more than 90 days prior to the requested date(s).
ALL ORGANIZATIONS MUST PROVIDE AN ACORD CERTIFICATE OF LIABILITY INSURANCE, LISTING CEDAR HILL ISD AS THE CERTIFICATE HOLDER AND ADDITIONAL INSURED, BASED ON THE REQUIREMENTS OUTLINED IN SECTION 2 OF THE SCHOOL BOARD FACILITY RENTAL TERMS AND CONDITIONS (EXHIBIT A):

1. Commercial General Liability (min. $1,000,000 per occurrence)
2. Automobile Liability (min. $500,000 per occurrence - if an organization is transporting anything onto school property)
3. Workers Compensation (min. $100,000 per occurrence - if an organization has hired staff or is contracting services)
4. Participant Accidental Insurance w/Excess Medical (min. $25,000 per occurrence - REQUIRED for all youth athletics)
