1. Please fill out the Contact Form at the beginning of each school year even if you filled it out the year before.
2. Please obtain correct parent email address and cell phone number before filling out the form.
3. Students please DO NOT use your school email address.
4. Enter email addresses correctly!!!
5. Your schedule may have a different name for your music class than the options on the form. Please select "Orchestra" if your schedule says Orchestra, Chamber Orch, Intermediate Orch, or Advanced Orch. Please select "Band" if your schedule says Band, Symphonic Band, or Wind Ensemble.
6. If a required field does not apply to you please enter N/A.
7. DO NOT FILL OUT THIS FORM MORE THAN ONCE. If you need to change your info, email the changes to
