7th Annual Play It - 4 on 4 2023 Registration Form
The event will take place on June 10, 2023  Games are anticipated to be scheduled between 8am - 4pm, the schedule will be confirmed and sent out closer to the tournament date. 
All players must supply their own equipment including gloves, helmet with face shield, and wiffle stick or hockey stick... shin pads are also recommended. No goalie equipment is supplied.

Please fill out this google form, once it has been completed with all questions answered click submit. One team payment will need to be sent via etransfer to edms.parentassociation@gmail.com. Please use the password 'dansereau' (all lowercase letters). In the notes please put the name of the person registering the team. Registration is not considered complete until payment has been received. The registration deadline is May 15, 2023. Late registrations may be accepted if there is room available. Once your team is registered and payment has been received, you will receive a link to the waiver form. Each participant will have to complete the form. 
What division would you like to play *
REGISTRATION FORM:
Register as a team (5-7 players is ideal, but up to 8 are allowed). Registration will be done based on birth year (YOB), not grade for
kids born from 2009-2016. Teams will be categorized into divisions based on registration numbers and competition level. A team can have a variety of ages on it, but older players may affect placement. All players must have waiver forms signed by a guardian prior to participating in this activity. Minimum three teams per category. In the event there are not enough teams registered, registration fees will be refunded in full.

VOLUNTEER REFEREES
We are actively looking for referees who would be paid an honorarium. Please contact Marcie Kagi at marcie.kagi@gshare.blackgold.ca

Each player will receive pizza for lunch. Concession will be available, bring cash.
Teams are encouraged to get creative and show their team spirit with fun team wear!! Their will be a team spirit prize. 
Custom Team shirts can be purchased by teams directly through She's Crafting in Beaumont. To order team shirts call 780-737-2738 or email shes-crafting@outlook.com. She's Crafting is located at #108 5305 Magasin Ave. in Beaumont.

Pricing: Reduced fee of $20.00 + GST per shirt (40% discount to EDMS Ball Hockey Teams)

Name Bars on shirts can be purchased for additional $8.00 + GST per shirt.
- Either the first or last name of players can be added to the back top of the team shirt
*Name Bars must be completed for all players on the team if this option is chosen.
*With this option you must provide She's Crafting with a list of players names with individual jersey size, team shirt color and logo design.
