Accompanying adults/carers do not need to complete a form, however please specify how many accompanying adults will be attending on this form.
This form is collecting information for the on-pitch activities as part of the festival. We will send out a further communication to collect information about the evening activities/events.
We are bringing together our Flyerz clubs to play hockey to celebrate 10 years of Wakefield Flyerz. We are also inviting local hockey clubs that don't currently have a Flyerz section and new Flyerz to try hockey for the first time.
Please note, you need to complete one form per child attending, including siblings and other family members.

The event is open to families of disabled and neurodivergent children and young adults. You do not need to have any experience playing hockey, and the event is inclusive of all abilities.
