Please read carefully before filling out this form!

**If you are not familiar with the mechanics of the burn, please visit our discord channel to get caught up.**

Every wine label that is burned will earn you 1 bottle of wine OR you can opt for a "Lagniappe Token" (see discord for utility of these tokens). For each red and white wine label you burn, you'll also get entered into a raffle for 10 NFTs that will take place on April 15th, 2022.  

**OTHER IMPORTANT NOTES**

#1 - The burn address for sending labels is wagmiburn.tez. ONLY send to this address.

#2 -  If you are receiving a DAO token, you don't have to specify on here. We will automatically be sending them based on the amount you burn. If you'd like to learn more about the DAO, check out our discord channel.

#3 - You can have your wine sent to ONE address per burn form. If you want to send you wine to other addresses, fill out another form. Please no more than 2 because it increases our shipping costs significantly.

#4 - We don't have an exact date to receive the wine. We are hoping to have them shipped by end of April. Please keep tabs in the discord.

#5 - If you are receiving a Lagniappe Token, they will be sent out the week of April 18th.

#6 - Everyone that burns a red and white label will get 1 entry into the raffle.
