Please report all notifications sent out by a school or district related to positive test results in a school. All personal information will be kept private. Only overall statistics and general information will be shared publicly. Submissions will be deidentified by removing names, email addresses, etc. but it is important to collect the information up front to allow for follow up. Iowans deserve to have their private information protected.
