PARENTS/GUARDIANS TO COMPLETE THIS FORM.

By completing you are giving consent to your child taking part in the full day of activities offered. If your child will not take part, or participates in a negative way, you will be contacted and asked to collect your child.

Please ensure you are contactable throughout the day and remain with your child for up to half an hour after the session begins to ensure they are settled before you leave them.
