Do you agree to be liable for any damage or loss caused by negligence, or for any other reason? *
Will you return the instrument to Ecole McTavish School at the end of the 2020/2021 school year, or sooner if your child does not finish the school year in Band class, in the same condition in which it was received? *
Have you paid your $100 annual rental fee on PowerSchool? (Please note this fee is non-refundable - the rental money goes toward repairing, cleaning, and replacing our instruments when needed.) *
