Volunteer Recognition Spring Submission Form (#505)
This online form has replaced the paper form #505 for all adult recognition presented in the spring by the service unit. To be filled out by SERVICE TEAM MEMBERS ONLY. 

After reviewing and approving all nominations and #501 forms received from volunteers in your area, complete and submit this order form for every candidate/recipient by April 1.  Update: For the 2023-2024 year, we will have two deadlines: All submissions received by April 1st will be fulfilled and mailed to service unit contact by April 15th. Submissions received after the 1st will be mailed on May 3rd.

Service unit teams determine eligibility for recognition categories and choose one member to submit all #505 forms. If a volunteer would like to nominate a service team member and keep the recognition a surprise, they may contact another member of the service team and have them submit a separate form. 

Recognitions submitted by April 1st will be delivered to service units by April 15th.

Years of GS Membership
The years of membership numeral guard is earned for the combination of both girl and adult years as a member of Girl Scouts. Service units are responsible for maintaining membership records for their volunteers. The council provides Years of Membership recognition pins at five-year intervals starting with 20 years. If candidate is receiving a Years of Membership pin, please enter # of years below (no less than 20, in five-year increments).
Years of GS Service
Girl Scouts of the USA offers recognition to registered adult members for years they have served as an active adult volunteer in Girl Scouts. Service units are responsible for maintaining service records for their volunteers. The council provides Years of Service recognition pins at five-year intervals starting with 20 years. GSUSA is now only recognizing whole year number increments after 35 years. If candidate is receiving a Years of Service pin, please enter # of years below (no less than 20, in five-year increments/10-year increments after 40 years).

GSOSW Leadership Development Pin - NEW as of 2023

This pin recognizes a volunteer who has completed required training for a troop or group setting. The volunteer must have completed the following trainings:

1.      Leader Orientation* as provided by the Service Unit (*required for those who are in the Troop Leader position only. )

2.      Grade Level Training – previously in-person and/or currently online

3.      Girl Scouts Inside and Out, Parts 1 and 2 – previously known as Day Trips and Indoor Overnights, which both qualify. More seasoned volunteers may have other training(s) by different names that meet the same training criteria.  These will qualify.

Service units are responsible for maintaining training records for their volunteers.

Outstanding Community Supporter
Recognizes outstanding contributions of time and/or services to local Girl Scout groups by an individual or business. This recognition is presented in the form of a certificate and can be awarded year after year for continued support. One letter of endorsement should have been submitted to the service unit. Check the box below if endorsement letter has been reviewed and the individual or business has been approved by the service team to receive an Outstanding Community Supporter certificate. Please make sure the Candidate's name above reflects the name to be printed on the certificate (needs to be less than 25 characters). If the nominator wishes to recognize the recipient for multiple years of service, please indicate the number of years in the Other box below and the certificate will be customized.
Outdoor Enthusiast
This recognition recognizes a volunteer who promotes and champions outdoor skill progression in Girl Scouts. Two letters of endorsement should have been submitted to the service unit. Check the box below if endorsement letters has been reviewed and the candidate has been approved by the service team to receive an Outdoor Enthusiast Recognition. Note: this recognition pin can now only be earned once. If the candidate has received it before but the nominator would like for them to still receive additional recognition, then other categories of recognition should be considered.
Owl
This recognition pin recognizes a volunteer who provides learning opportunities for girls and/or adults in Girl Scouts. This volunteer does not have to be a member of the GSOSW Trainer Corps. Two letters of endorsement should have been submitted to the service unit. Check the box below if endorsement letters have been reviewed and the candidate has been approved by the service team to receive an Owl pin. Note: this recognition may be awarded multiple times in subsequent years, but it must be for different accomplishments or acts of service.
Rising Star
Recognizes the service of an individual who is new to Girl Scouts and has hit the ground running and performed outstanding work within their first two years as a volunteer. Two letters of endorsement should have been submitted to the service unit. Check the box below if endorsement letters have been reviewed and the candidate has been approved by the service team to receive a Rising Star patch.
Guiding Star
This recognition pin recognizes a volunteer whose performance mentoring troops/groups merits recognition by the service unit. Two letters of endorsement should have been submitted to the service unit. Check the box below if endorsement letters have been reviewed and the candidate has been approved by the service team to receive a Guiding Star pin. Note: this recognition may be awarded multiple times in subsequent years, but it must be for different accomplishments or acts of service.
