Community Student Learning Sites Survey
Polk County Schools is working to partner with local community agencies to potentially provide local learning spaces for students in middle or high school who are learning remotely, or who need internet access or support on days when they are not at school.  For example, a local church might offer their fellowship hall to provide a safe space with internet access for students to come and complete schoolwork and receive learning support from volunteers.  These sites will follow all current health and safety guidelines, and all volunteers would be screened and background checked.  We are sending this short survey to gauge how much student and family interest there might be in utilizing these spaces.

If you are not interested in taking advantage of these sites and services you should not complete this survey.  If you are interested or would like to know more please read the prompts and respond to the best of your ability.  Thank you in advance for your participation.
