Isle of Faith Vacation Bible School registration form
Dates: June 17-20, 2019, Monday-Thursday from 6-8:30pm

FREE VBS for children who will be entering K-5 in Fall of 2019.
(Please note that children who just finished 5th grade will not be eligible for the Isle of Faith VBS program.)

NOTE:
1.) Please complete one form per child.
2.) Dinner will not be served at VBS, however a light snack will be part of each evening.
