This is an ‘Application to Train’ google form. This form must be completed by all swimmers who wish to return or continue to swim and those taking a break in Term 4. The form begins with ‘Are you training this term?’

If 'TAKING A BREAK', the form will submit and end. Thank you for informing us.

If 'Training' your request will be processed and an invoice will be generated depending on your responses to the number of training sessions selected. Athletes can not train unless this form is completed.

TERM 4:
Emerald Junior School Squad: 7 October - 2 December  (9 Weeks)
Bronze Squad: 26 September - 17 December (12 Weeks)
Silver Squad: 3 October - 17 December  (11 Weeks)
Gold Squad:  3 October - 17 December  (11 Weeks)

On this form you will indicate which sessions you are attending for the term:
*Athlete's may make up a missed session within the week unless arranged prior to the session. Max: 2 per term

*Invoices for the training periods will be sent once you indicate the number of sessions your child is planning to attend per week on this form.
*Invoices need to be paid by the due date
*Complete one form for each athlete
