The purpose of this form was to provide a means to submit a formal request to obtain a COVID-19 vaccine exemption for medical or religious reasons regarding attendance of Interfusion Festival. Person who had either (1) a medical condition or disability that prevented them from taking a COVID-19 vaccine or (2) a religious observance that prevented them from taking a COVID-19 vaccine were entitled to request an exemption. -Health & Safety Team
