Please read the following information closely. All personal information that you will be asked to provide will be used solely to help improve future CPT (40 hour) training's and will not be used (or shared) for any other reason.

ADMISSION
Applications will be considered on a first come, first serve basis. Our goal is to make this program available to as many people as possible. However, in order to keep participation and learning engagement high, enrollment in our programs will be limited to 15 students and a 5 student wait-list. Enrollment for the program will close once all spots have been filled; but no later than Monday, April 13, 2020. Individuals who have been selected to participate in the program will receive an acceptance email message the week of April 15, 2020.
