SPPS Device Repair and Troubleshooting Form
Students, Parents, and Guardians,

This form is used to report an issue with your school issued device (iPad or Chromebook). Once the form has been submitted, a school district employee will reach out regarding information about device drop-off, repair and subsequent pickup when the device is repaired or replaced.
