Families, please complete the form below indicating if your student(s) will be utilizing the hybrid schedule or 100% remote learning.

Please submit before 3:00 p.m. on Friday, October 23rd.  The remote learning option will continue through January 15th, the end of the second quarter at which time families can re-evaluate their decision.
