I request that the above applicants be accepted and agree to abide by all rules and regulations of Acme Pro and otherwise undertake to behave in such a manner as to contribute to the safety and well-being of myself and others. I understand Acme Pro assumes no responsibility for injuries or illnesses which I or my family members may sustain as a result of my/our physical condition or from my/our participation in any of its activities, use of venues, equipment and facilities, and expressly acknowledge on behalf of myself / my family and my heirs that I assume the risk for any and all injuries and illnesses which may result from participation in these activities, use of its venues, equipment and facilities. I hereby release and discharge Acme Pro, its servants and employees from any and all claims for injury, illness, death, loss or damage which I, my family members or my pet may suffer as a result of participation in these activities, use of its venues, equipment and facilities.
