5th Grade Walkover Permission Form 2023
Dear 5th Grade Families,

We are excited to announce the 5th Grade visit to the Middle School this year!

Instead of sending a paper permission form we are asking families to complete this Google Form giving electronic permission to allow their child/children to attend this important transition to Middle School experience.

Each class is walking over this year (no busses) at a different day and time as indicated below.

March 27, 5SC (Ms. Combs) 10:00- 12:00 pm
March 27, 5JT/JB (Ms. Trotta & Ms. Benvenuto) 11:45 am- 2:00 pm
March  28, 5SG (Ms. Golio) 10:00- 12:00 pm
March 28, 5PB (Ms. Burstein) 11:45 am- 2:00 pm
March 29, 5LW (Ms. Wolman) 10:00- 12:00 pm
March 29, 5AE (Ms. Eklund) 11:45 am- 2:00 pm
TBD...will be a rain date for any of the classes

There will be a tour of the building, a Q & A session with current sixth graders, and students will be able to enjoy lunch from the Dining Hall

                   ◆Students will enter their Student ID number at the register to receive a free lunch.
                   ◆Lunch includes fruit, milk or juice and either the lunch special of the day, pizza or the pasta special.
                   ◆Students may bring lunch from home in a brown bag--no glass bottles, please!
                   ◆Students may bring extra money to purchase snack foods.

If you have two or more siblings in the 5th grade, you must complete the form separately for each child in the grade. Thanks.
