This is the first of a two-part assessment process, which if successfully completed will result in the applicant being signed to Angelbee Nursing Service, henceforth referred to as ANS, as a Home Nurse or Home Caregiver.

The entire process will be completed online and is open to nurses and caregivers anywhere in Jamaica as it will not require an in-person visit.

If you are already signed to Angelbee Nursing Service, you are still being asked to complete this process, which will result in your profile being moved online and available in an app that will now be used to select persons to be offered jobs as the old database will no longer be used.

Please be reminded that this does not constitute an offer of a job. Successful completion of this process will result in you being listed as available to provide nursing care if you are a nurse or caregiving if you are a caregiver, in private homes.

The offer of a job will only occur if a client request a nurse/caregiver and you are the most suitable candidate by virtue of your skills, experience, location, etc. For more information on placement, please log into our website as indicated below. Please log into ANS's website at https://www.angelbeenursingservice.com/ and read "Covid-19 Safety" and "Staff Code of Conduct" before starting this assessment as you will be asked to answer questions about these two posts.

Also, If you do not already have a Google e-mail address (@gmail), please create one now as one will be required to access the following pages of this form; to submit your supporting documents and for you to receive the quiz, which represents part 2 of this process.

Please use the same e-mail address that you plan to use for the second part of this process as part 2 of the assessment as well as other communication will only be sent to you via the e-mail address that you list here.

You will be asked in Section 3 - DOCUMENTATION, to indicate which document you possess. Once you indicate that you possess a document, a copy must be sent to ANS except your NIS; Your TRN, and your Driver's License with your TRN. For security reasons these will be requested at a later date so please do NOT tack them anywhere on this form as we cannot guarantee the safety of that sensitive information in this medium.

Please scan or take clear photos of all your supporting documents to be sent (except the three mentioned above), via e-mail to angelbeens@gmail.com, within 72 hours of submitting this form as the documents are required for your application to be processed. Hold the three mentioned above until they are requested.

If you do not already have a Food Handler's Permit, please make the necessary arrangements to get one as this is now a requirement of the Ministry of Health and Wellness for anyone providing personal care and or food service to anyone who is not a family member.
