Mountain Day Permission Form and Volunteer Driver Sign Up

The Mountain Day tradition continues!  We are moving to an online permission form, below, and also recruiting parent volunteer drivers for grades 9, 10, and 12. Please read and fill out the form below. (To learn more about TA's Mountain Day tradition, click here.)

Field Trip Permission Form: 
Thetford Academy is a non-profit educational institution. References to Thetford Academy (henceforth referred to as TA) include its trustees, employees, volunteers, participating organizations, agents and assigns.

I understand that: 

  • TA is not an agent of, and has no responsibility for any third party including any organization which may provide services including food, lodging, travel, or any equipment associated with this Mountain day.

  • Despite precautions and supervision by TA, accidents or injury may occur in the course of participating in this activity. I assume ALL risk associated with the above noted student’s participation in Mountain Day on September 23, 2022. 

I acknowledge that I will not seek to have Thetford Academy held liable in the event of an accident, injury, loss of property or any other circumstances or incidents occurring during or as a result of participation in the outing referenced above. I hereby release and agree to hold harmless TA from any claims arising out of participation in this event. 

I authorize TA to act on my behalf in any medical emergency.

Thetford Academy welcomes and expects all students to attend Mountain Day, but we understand some may not be able to. Check below if your student will not be participating. (Please note: No students will be allowed to remain on campus that day.)
