2023 Middle School Prom Volunteers

This form is for adults interested in being a chaperone or volunteer at the Willard Parks Middle School Prom on April 28, 2023 6-8pm at the Willard Recreation Center. Volunteers may be subject to background checks.

Chaperone/volunteers' middle school-age children receive free entry ($5 value) into the dance!
