This form needs to be filled out prior to the end of the year.

Directors are asked to do the following before filling this form out.

1. Use inventory & pre-registration data to determine instrument needs for the program
2. Identify which instruments typically are not available to rent through a local music store
3.Enter those instrument needs for the upcoming school year (by school) on this form.
4. A copy of your submissions will be emailed to you for your records.
5. Throughout the summer follow up with me on whether we can get those instruments.
