This form is for those looking to register for the 2021 Student Leadership virtual conference and pay by check.  The link to pay electronically can be found at https://registration.socio.events/e/slc

Registration cost is $20 per person for OTA Members.  (Haven't paid your 2021 membership yet?  Instructions for payment can be found here: https://drive.google.com/file/d/1ccLssfsibGieUwX2ynVzCDrOKezbpIUb/view?usp=sharing).

Please complete this form for each individual you will be registering for the conference.  You can also email a spreadsheet with your list of participants answering each of the 4 questions below.  See the contact info above for questions regarding collecting student information and payment.  An email with information to access Socio and the full virtual conference will be provided to the students as we get closer to the event on May 5th.

Payments can be grouped, no need to send checks for each registrant.  Please use the following instructions for mailing a check.
