Thank you for choosing THE UPPER DECK to provide you with the tools/equipment to keep you healthy and fit! Here are the step-by-step procedure to rent our fitness equipment:

1. Completely fill out this form so we can process your request;

2. Expect to receive a message regarding your order within 5 working days from the date of accomplishing this form;

3. If your application is pre-approved, we will send you a printed copy of the Equipment Rental Agreement, the list of other requirements (proof of identification and proof of ownership /contract of lease) that you have to submit and the complete address where you have to send back the originally signed copy of the Equipment Rental Agreement. The delivery fee of the documents shall be shouldered by the borrower. Please let us know if you prefer printing the document at home, so we can just send you the soft copy of the Equipment Rental Agreement;

4. As soon as we receive your complete requirements, we will conduct a video call with you to further complete the processing of your application;

5. As soon as your application is approved, you will receive an email containing the official invoice and online payment instructions so you can settle your bill; and

6. An acknowledgment of your payment and the delivery information will be sent through email.
