The Sherman Library will not share your minor's name or private information. You, the parent/caregiver whose name has been provided on this registration form, give your consent for the Sherman Library to use your child's photograph and likeness by checking the choice/s below. You, the parent/caregiver, release the Sherman Library from any expectation of confidentiality for the undersigned minor child and attest that you are the parent or legal guardian of the child registered on this form.
